When should I schedule my appointment?
- If you have a PPO insurance plan, you are not assigned to a doctor and you can go to any network provider and make your appointment anytime.
What if I have a HMO?
- You would first have to contact your insurance company and choose Dr. Patel as your primary care physician. Then you could call and make your appointment any time.
When I call to make my appointment what information do you need?
- If you have access to a fax machine we ask that you fax a copy of your drivers license and front and back of your insurance card. If not, you can drop by and we will make copies for you.
What if I don't have my insurance card yet, but have my old card with the old information?
- Well, you can either call your insurance company and have them fax us something that indicates information on what your co-pay is and what PCP you were assigned and what your effective date is. This way it will be quicker for your office visit, or you can come in for your office visit and wait a little longer while we verify the insurance.
Can I pickup my paperwork, so I can have it ready at the same time as my appointment?
- Yes, you can pickup your paperwork or we can fax it over to you.
I was given Dr. Patel's name for my Immigration Exam, can you give me some information?
- Yes, the exam price is $140 dollars and includes the PPD, blood work and exam with the doctor. If your PPD becomes positive the x-ray is included in the exam.
Is this the only thing that I.N.S. requires?
- No, depending on your age, they do require current immunizations. You can look on our graph in our office to see that information. If you have had your immunizations and have your card that is stamped by the physician who gave them to you, you can bring them in for the nurse to look over.
